Am I Ready to Stop Therapy?

I got my first hint that I might be ready to stop therapy when I realized how little I was going. Over the years I have scaled down from weekly sessions to biweekly.

Then I noticed that, effectively, I’ve been going only once a month. I’ve been forgetting appointments, showing up on the wrong day, oversleeping, feeling poorly physically, or having too much freelance work to do.

Of course, those could be signs that I’m in denial, that I’m resisting therapy, that we’ve hit a bad patch of difficult issues and I just don’t want to deal with them.

But I don’t think that’s what’s happening. Here’s why.

I’m stabilized on my medications and they’re effective. When my psychiatrist moved away a few months ago, he left me with enough refills to last until this month and a list of other psychiatrists. My PCP agreed to prescribe my psychotropics if I lined up another psychiatrist for emergencies. I’ve done that, though I couldn’t get an appointment before March.

And that doesn’t alarm me. I don’t have the oh-my-god-what-if-my-brain-breaks-again panics. I don’t have the feeling that my brain is about to break again. I’ve thought about it, and I’m comfortable with letting my involvement with the psychiatric profession fade into the background of my life.

As long as I keep getting my meds.

I have more good days and I’m beginning to trust them. Oh, I still question whether I’m genuinely feeling good, happy, and productive or whether I’m merely riding the slight high of hypomania. But really? It doesn’t seem to matter very much. A few days ago I reflected on a string of particularly good days – when I accomplished things, enjoyed my hobbies, and generally felt content. And I simply allowed myself to bask in those feelings.

That’s not to say I don’t still have bad days. After a few days of hypomania, I hit the wall, look around for spoons and don’t find any, and require mega-naps to restore me. (I’m intensely grateful that I work at home and can do that. Most offices don’t appreciate finding an employee snoring underneath her desk. And my cat-filled bed is much more comfy-cozy.)

I still get low days too, but they are noticeably dysthymic rather than full-out, sobbing-for-no-reason, Pit-of-Despair-type lows that last seemingly forever. I know – really know, deep within me – that they will last a day or two at the most. And just that knowledge makes me feel a little bit better.

My creativity, concentration, and output are improving. I can work longer, read longer, write longer, take on new projects, think past today or even next week. I can trust my muse and my energy, if not immediately when I call on them, at least within a reasonable time.

I have trouble remembering how bad it used to be. Recently I’ve made connections with several on-line support groups for bipolar and mental health. I find I’m astonished at the crises, the outpourings of misery, the questioning of every feeling and circumstance, the desperate drama of even the most mundane interactions. They are overwhelming. But I realized that it’s been a long time since they’ve overwhelmed me. I recognize that I could some day be in that place again – that’s the nature of this disease. But I have a good support system that I trust to help me not fall too far without a net.

I don’t have much to talk about when I go to therapy. There are issues I need to work on – getting older, getting out of the house more, reclaiming my sexuality. But most of those I feel competent to work out on my own.  My sessions are mostly an update on what’s going on in my life at the moment, plus a recap of my recurring problems. But those problems are ones I’ve faced before and know how to cope with. I already have the tools I need and use them without needing a reminder.

So I’ve talked it over with my psychotherapist and I’m not completely quitting therapy, but I am cutting back officially to the once a month I seem to be going anyway. I know that if and when the bipolar starts giving me major trouble again, I can always call for an appointment or a telephone therapy session.

I’m not going to stop writing these posts. I still have a lot to say about where I’ve been, how I’ve got to where I am now, how things will go in the future, and all the many ways that mental illness affects society and vice versa.

You’re not getting rid of me that easily. I’m sticking around.

Going Public

I just posted this on my Facebook page. Now we wait and see what happens.

Along with the news of Robin Williams’s death have come discussions of mental illness and suicide. I’ve decided to go public with my own experience. I have bipolar disorder – type 2 (which means that I have lots of depression and anxiety, but very few manic phases). I’ve had this all my life, most likely, so whenever you met me, I had it.

Some of my friends already know and I’m sure others have guessed or suspected it. It is the result of a biochemical imbalance in my brain and is now treated with medication and therapy. I’m working on it.

Anyway, I ask for your understanding when I sometimes go hide under a rock for a while or say or do something odd or rude or unkind. My social skills have never been great, and having a disorder like this doesn’t improve them. I’m working on that too.

But you don’t have to do anything special or tiptoe around me. I’m still who I always was. I don’t freak out when people call me crazy or nuts or weird.

Hired Help

Remember when you go to a therapist, you hire him or her. She/he works for you. And if it doesn’t work out you can fire your therapist.

I hate breaking in a new therapist. I hate having to do the little tap dance that is the Reader’s Digest Condensed version of my screwed-up life. But if I have to find a new therapist, I’ll keep reciting it until I find the right one.

Of course your choices may be limited by location, finances, or other factors (EAPs, for instance). You may not have that wide a range to choose from. But the better the “fit” between you and your therapist, the more likely you’ll make some progress.

When I need a new therapist (when mine has moved away, for instance) I go through a process.

Step One: I identify my current needs. Do I need counseling for grief? Mood disorders? Anxiety?

Step Two: I do research. Most therapists have websites and most of them list what they specialize in. Start making a list of those whose practices meet my needs.

Step 3: I make a list of my preferences. For me, this is easy. By now I know I need a therapist who is not Freudian or cognitive behavioralist or comes from any particular religious approach, but does deal with women’s issues and has a sense of humor.

Step 4: I make some calls. Is the therapist taking new patients? Is there a waiting list? Does he/she take my insurance? If not, is there a sliding fee scale based on need? Is long-term therapy possible, or is it 30 days max?

Step 5: I set up an “interview” appointment. I do the even-more-condensed version of my screwed-up life, ask a few questions and, if everything “feels” pretty good, make a follow-up appointment.

I know, that’s a lot of work. But in the long run, you can save yourself a lot of grief.

I’ve been to a couples counselor who absolutely shredded me, latching on to my husband’s difficulties with me and running with them. I felt she didn’t hear anything I said, or if she did, discounted it. I felt ganged up on. It was not pleasant. More important, it was not helpful.

(I’m not saying that she should have ignored my husband’s issues, which were important and valid. But I really needed to be navigating on a two-way street.)

Another therapist I went to decided that my problems stemmed from being an innocent led astray by an older man (ten years older) who got me into practices that went against my upbringing. Which is not the way it was at all. (It was the 70s. Some experimentation with sex and drugs for a woman in her 20s was not considered total depravity then.)

She also diagnosed me with PTSD because I said I had flashbacks and dreams about the bad parts.

In both of those cases, I got the hell out.

And that’s my point. I had a choice and I exercised it. I went looking for someone else who could help me with my problems. And I found a psychotherapist who could and has, plus a psychiatrist for my meds.

I’m not saying you should ditch your therapist when the process hits uncomfortable or even painful patches. It’s pretty much got to if it’s going to help.

But you made the choice of hiring this person. You also have the choice – and the right– to leave and go looking for someone else.

Risky Business

It’s always a risk when you admit publicly to having a mental disorder. But I am thinking of doing just that.

I have not had uniform success when I have revealed to others that I have bipolar disorder (or chronic depression, either). There have been a lot of “me too’s” and “so’s my brother/sister/mother/friend/etc.” and then we compare diagnoses and symptoms and meds and war stories and have a jolly time.

Other times, well… My mother hoped my problems would go away after I got a “good, steady job.” My father said he didn’t mind if I went to a therapist “as long as he didn’t have to go too.” My mother-in-law “doesn’t believe in mental illness.” My rotten-ex-boyfriend “jokingly” suggested that if we went to couples counseling, he and the therapist could agree that I was a danger to self and others and have me put away. (I knew that wasn’t true and told him so. We went. It didn’t help.)

Recently I have started two blogs, this one for mental health issues and a more general one called Et Cetera, etc. (which you’re welcome to visit if you like). I have linked Et Cetera to my Facebook account, but so far I haven’t linked this one.

Starting these blogs feels like a risk to me, especially since I’ve set WordPress to remind me to post at least once a week. Making a commitment that I will pull myself together four times a month (eight if you count the other blog) and write is something I’m not completely sure I can do. I have good days and bad days, and sometimes those bad days pile up in a bunch.

But I have also taken a bigger risk. Creative Nonfiction magazine requested submissions to be considered for its Mental Health Anthology. So I submitted one. If it gets chosen, I will be “coming out” as bipolar and a mental patient. When (if) that happens, I will likely do the “big reveal” on Facebook. A fair number of my close friends already know, but they constitute only a small segment of my FB friends.

(Creative Nonfiction accepts only manuscripts that have not been published elsewhere. If they don’t accept mine, you can bet it’s going up on this blog the next day.)

So, having thoroughly terrified myself, I will anxiously await the results. And in the meantime, I’ll try to keep up the regular blogging.

Brain Hamsters, Stomach Badger

I’m sure you all are way too familiar with the brain hamsters – those little buggers who spin your wheels whenever you try to fall asleep. It’s not a new phenomenon, a new concept, or even a new name for it.

But now the brain hamsters have their very own song. My friend Leslie and I, plagued by the little rodents, used to end our phone conversations, “Death to hamsters!” This inspired our friend Tom to write a dotty little ditty on the subject. It has become a popular sing-along in certain circles – just imagine a room full of people all chorusing, “Death to hamsters!” It’s positively inspiring.

There is, however, another inner animal that has plagued me.

My last full-time job caused me a great deal of anxiety. Monumentally so. My boss left, and I felt I should tell my new boss about my depression (not diagnosed as bipolar yet). She said, “What does that mean?” Uh-oh. My stomach sank, and the badger moved in.

I missed a lot of time at work dealing with my mother’s failing health and finances, in addition to my own. She was blown over by a gust of wind, fell like a plank, and landed on her face. A neighbor sent her kid over: “Go see if she’s alive.” I had to have the you-can’t-live-alone talk with her. Find a nursing home. Figure out how to pay for it. Et cetera.

I could feel the stress in my stomach. A nasty badger, red in tooth and claw, growing daily, snarling more loudly, and threatening to claw its way out. Like that scene in Aliens, except an angry anxiety badger instead of a nameless whatever-that-was.

There was one good thing about the badger. Its presence alerted me that it was time to get the hell out. So I quit my job to go freelance. And it worked. For a while. I remember feeling happy, feeling free, as I drove on my errands and worked at my own pace and on my own schedule.

Of course it couldn’t last. The badger was only lying low, waiting for another round of minor and major disasters to resurrect it. And they came. My, how they came.

Then the badger won. My brain broke. I’ve been trying to piece it back together ever since. Thanks to my support system, my doctors and my medications, I am slowly doing so.

But the badger is waiting. I can feel it stirring, even now.

If you don’t already know about The Bloggess, well, you should. And here’s your chance. I especially like the advice about seeing a shrink. Remember: You are hiring the therapist. You can look for another one if needed. I had one that said I had PTSD because of some non-standard sexual experiences. And a couples therapist who shredded me and left me worse than when I came in. I learned to interview therapists before I made the first appointment. Do you deal with women’s issues, mood disorders, grief counseling? (Good.) Are you a Freudian, a cognitive behavioral therapist, a religious counselor, easily offended? (For me, at least, not so good.) And do you have a sense of humor? Then I give it a couple of visits to do the Reader’s Digest condensed version of my screwed-up life. By then I can usually tell if we are right for each other. And if we’re not, I MOVE ON. And keep looking. I need a professional therapist in my support system, but not just anyone will do.

I don’t know if it’s the planets or the meds or the darkness of winter, but this week I’ve been a bit down in the hole and I suspect I’m not the only one.  Then I heard this song that I’ve loved and forgotten and it saved me a little bit.  Little things save me from myself all the time.  Sometimes it’s music, and sometimes it’s words from writers who’ve been dead for years, and sometimes it’s you.

If you’re sad or lonely or feeling like you’re one of the misfit toys, know that you are part of us.  And remember that those misfit toys were always far more interesting than the normal ones.

Tell someone that you love them, or that they’re important.  And tell yourself.  Because it’s true.
